8.Blood bank license or outsource Mou with blood bank.
9. canteen MOU
10.Special Radiology and pathology test mou with outsource diagnostic centre.
11.BMW disposal authority MOU.
12. AERB for C Arm, CT scan, Mobile X ray, fixed X ray, OPG machine according to availability.
13. TLD badges for OT staffs, minimum 2, Radiology staffs minimum 2,;CT and MRI staffs minimum 2, OPG staffs minimum one. Depends upon the availability of diagnostic services and availability of C arm in OT.
13. Drugs and cosmetic act license ( Drug license)
14.Narcotic drugs license.

16. STP and ETP available for BMW air and water license.
17.Fire NOC.
18.FSCI license for canteen.
19.RSO (Radiation and safety officer license from AERB)

24. Important and emergency instruments and equipment available according to deptt wise (List should be available deptt wise)
25.Medical record department with computer and printer and scanner available.
26. One room desinated for empanelment with all computer and printer scanner and all.
27. Special design stickers and codes , Directory of hospital sticker floor wise deptt wise, direction wise , fire escape stickers should be pasted strategic location.
28. HIS system should be available. All billing should be computer generated.
29.Hospital services stickers should be available front of the hospital.
30. Pharmacy counter with LASA and emergency drugs pigeon hole should be designated.
31.All stationary and register pertaining to medical records properly documented by nursing, paramedics, specialist doctors, full signature of doctors properly embossed in every visit and stamps with registration no. Nursing note should be available,

OPD, Emergency, ICU, NICU, wards every where hand washig facilities should be available.
Modular OT required for NABH accreditation not for entry level nabh accreditation size 22x22
CSSD room size minimum 12x12 with dirty and clean window and should be well ventilated.
X ray room size minimun 12 x 14 sq feet with printing and reporting space, store space.
CT scan room
Console room minimum 10x8 sq ft
Genitor room, : 14 x20 sq ft.
Ultrasound room 12x10 with wash room is just near by area.
